Test Method Principle
The Diagnostic Automation/Cortez Inc. Neonatal TSH assay is a “sandwich” type of solid-phase enzyme immunoassay, based on two monoclonal antibodies with different epitope specificity to β-subunit of TSH molecule. One of these antibodies is conjugated with enzyme; the other is conjugated to biotin and immobilized on the surface of streptavidin coated microplate. TSH molecules from the sample bind to both immobilized antibody and anti-TSH-enzyme conjugate. Then the wells are washed with wash buffer to remove any material not bound on the inner surface of the wells. Quantity of the bound conjugate is directly proportional to the TSH level in neo-natal sample.




Background Information
TSH is secreted by the anterior lobe of the pituitary gland and induces the production and release of thyroid hormones. The thyroid hormones exert a negative feedback on the pituitary. The release of TSH is regulated by TSH-releasing hormone (TRH) produced in the hypothalamus. When there are high circulating levels of thyroid hormone in the blood, less TRH is released by the hypothalamus, so less TSH is secreted by the pituitary and vice versa.
Determination of hypothyroidism within the first few days of birth has been recognized as the single most important diagnostic test in neonates by the American Thyroid Association. Congenital hypothyroidism is probably the single most common preventable cause of mental retardation. Diagnosis and treatment of congenital hypothyroidism within the first 1-2 months after birth appears to be necessary in order to prevent severe mental retardation. Measurement of the serum concentration of TSH is generally regarded as the most sensitive indicator available for the diagnosis of primary and secondary (originated from pituitary gland) hypothyroidism. Increase in serum concentrations of TSH, is an early and sensitive indicator of decrease thyroid reserve and in conjunction with decreased thyroxine (T4) concentrations is diagnostic of primary hypothyroidism. The expected increase in TSH concentrations demonstrates the classical negative feedback system between the pituitary and thyroid glands. That is, primary thyroid gland failure reduces secretion of the thyroid hormones, which in turn stimulates the release of TSH from the pituitary.
